when I create a form and place it on my website, the form signs and everything looks normal. However, I never relieve a notification that the form has been signed.

Thanks for getting in touch. We’re sorry to hear that you’ve some issues in receiving the E-mail notifications for signed Signature forms.
I’ve confirmed this issue and raised a ticket “SUPP-256” to the Development Team.
It will be very helpful if you could provide some additional info :
1. Your GroupDocs account - username.
2. The Production server used, for eg. apps.groupdocs.com
